Q: How do I obtain a copy of my high school transcript?
A: Former students may request a copy of their high school transcript by completing the “Former Student’s Application For School Records” then mailing it in with the required fee (as explained on page 2 of the form - General Instructions). Your application, once received, will be processed as soon as possible. Please allow a turnaround time of at least three weeks.
When sending your request, please be sure to include:
- Name used while in school
- Date of Birth
- Name of last Philadelphia public school attended with approximate year
- Address where transcript is to be sent
- Phone number where you can be reached during the day
- Required fee – money order or cashier’s check only
- Signature
The School District of Philadelphia maintains records for public schools only. Students who attended private, parochial, religious, or charter schools should contact those schools directly to obtain transcripts.
